Outreach

Our outreach teams search the streets and other areas in several counties we serve, looking for people experiencing homelessness.

We engage those individuals so we can connect them to housing, health care, mental health treatment, and other services.

Alameda County

Street Outreach Team

This team searches for and connects with people who live on the streets. When our teams connect with those in need, they provide service linkages to those participants throughout most of Alameda County.

Outreach Team

Our four-member Outreach Team engages and assists homeless individuals in the field, bringing housing and support services to people throughout Napa County wherever they are living — camps, cars, doorways, RVs, or staying temporarily with friends and family.

Outreach Team

Our five-member team provides intensive outreach and targets high-needs people, such as those with mental-health challenges. We engage and get them into a queue for housing and connect them to other services while they wait for housing. We also have an In-Home Outreach Team.

Street Outreach

Our Street Outreach team connects individuals experiencing homelessness and a behavioral health condition to a wide range of community resources and services.

Our team members also help thoe individuals by addressing immediate needs to promote well-being on the streets.
