Project Independence (PI) helps youth, from 18 to 24, who have aged out of the foster care system, and their children by providing them with affordable housing linked to comprehensive supportive services. This housing plus services strategy is effective because it provides youth with a stable foundation and adult support while they finish their education or job training, find new employment and/or overcome psychological problems that interfere with their ability to live independently. Wellness Connections Mental Health Clinician
Wellness Connections is a partnership between Tri-City Health Center and Abode Services. In alignment with the 2014 Healthcare reform, Alameda County has created programs to fully integrate primary care and mental health services for low income and/or homeless clients. To this end, the Wellness Connections program has been created. This team will help clients establish a healthcare home, obtain benefits and develop a long term relationship with both a primary care physician and a psychiatrist (if needed). The primary goal of this program is to help clients increase stability in health, housing, finances, social supports, and other daily living areas, decrease use of emergency services, and obtain benefits (SSI, SSDI, Medi-Cal, and/or Medi-Care). This position will provide specialty mental health services to clients meeting specific diagnostic criteria as set forth by the ACBHCS and Medi-Cal.
